GUIDE (Guiding an Improved Dementia Experience) is an 8-year CMS demonstration project offering comprehensive virtual care, 24/7 nurse access, and up to $2,500 per year in reimbursable respite services per eligible patient. Patients must have Medicare A & B and a dementia diagnosis (or suspected impairment), but they can still receive Medicaid waiver, VA, or private pay services concurrently.
"This is care management for the masses," said Nancy Gillette. “Families are utilizing the hospital less and staying at home longer because they now have clinical support on demand.”
The program leverages a clinical overlay model where PocketRN handles all virtual assessments, documentation, and billing — while agency partners provide hands-on respite care and home safety evaluations. Nevvon supports the initiative through flexible, on-demand caregiver training that reduces burnout and improves family engagement.
Allan Levine emphasized the urgency of adoption: “There’s a dementia tsunami coming. Providers have to do more with less — and GUIDE is a meaningful way to expand your impact without overloading your team.”
The discussion also covered onboarding, billing, eligibility, and how GUIDE seamlessly integrates with existing agency workflows without disrupting private pay, Medicaid, or VA revenue.
